An experienced Adults Social Worker is required to support clients in the Transitions team. To manage a case load and complete assessments, support plans and reviews with adults. Experience of working with challenging client groups is essential for this role.
Key Responsibilities


Manage a case load and complete assessments, support plans and reviews with adults.
Complete Court of Protection documents, best interests and safeguarding investigations.
Assess risk, explore suitable placements (Supported Living, residential and housing) alongside health professionals.
Support the team and to ‘hit the ground running’ with experience and knowledge of working with challenging client groups.

Qualifications:


Social Work qualification (MA/BA Honours, DipSW, or equivalent) and ASYE fitness to practice certificate.
Current registration with the Health Care Professional Council (HCPC) maintaining registration and adhering to HCPC standards and code of ethics.

Knowledge:


Sound knowledge of relevant legislation.

Experience:


Experience of working with challenging client groups is essential.
Significant experience in statutory services with strong communication, engagement and analytical skills to allow you to safeguard and engage effectively with a wide range of agencies.
Liquid Logic experience - preferable
Experience of writing high standard reports and written records for a variety of purposes including those in a legal context.
Experience of managing a high level, complex and diverse caseload within agreed parameters, policy and procedures.
Experience and skills in multi-agency partnership working.





Skills & Abilities:


Strong IT skills including Microsoft packages or equivalent (e.g. G Suite).
Ability to prioritise work with competing demands, frequently changing demands.
Ability to communicate, negotiate, challenge and manage conflict, acting respectfully when people are resistant to change, managing tensions in ways that show persistence, determination and professional confidence.
Ability to work as an autonomous professional, exercising own professional judgement using written guidance and established procedure, recognising one’s own professional limitations and how and when to seek advice from a range of sources, including practice supervisors, senior practice leaders and other multi-agency providers.
A high level of effective written and verbal communication and the ability to pass information accurately and concisely with a range of partners.
Able to apply consistently high standards to ensure better outcomes.
Self-motivated, innovative and creative.
